What causes shoulder pain?
Shoulders are some of the most complex parts of our bodies—an intersection of several joints that combine with tendons and muscles, allowing us an incredibly diverse range of motion. As a result, shoulders often become a problem area due to:
- Tendon inflammation or tendon tear. Activities that put a lot of pressure on our shoulders, frequently, are among primary causes for bursitis—an inflammation in the bursae—and tendonitis an inflammation in the tendon. With extreme trauma, a tear of the tendon is possible, too, resulting in sharp pain during motion and shoulder instability.
- Shoulder instability. Another common cause for shoulder pain is the looseness between the ligaments, tendons and muscles around the shoulder. Instability is as likely to be caused by gradual shoulder overuse as by a sudden trauma.
- Arthritis. The most common type of arthritis found in shoulders is osteoarthritis, otherwise called the “wear and tear arthritis.” As the name suggests, the condition is mostly related to your age, with the symptoms of swelling, stiffness and pain starting to show during middle age.
- Fractures.In rarer cases, the pain in your shoulder could be caused by a fracture. This is, however, fairly easy to diagnose, as the pain is sharp, and the bruising and swelling around the shoulder area are pronounced. Fractures usually happen as a result of an unsuccessful fall from standing height.

What other options do I have outside of surgery?
Previously, it has been thought that surgeries are the only way to ease your shoulder pain. New evidence suggests otherwise:
- Subacromial decompression, a popular form of surgery aimed to tackle chronic shoulder pain, has been found to be no more effective than placebo treatment by the University of Oxford professor David Beard
- A similar study by the University of Helsinki with 189 patients has confirmed that placebo treatment and physical therapy are just as effective as one of the most common shoulder surgeries, in the world
- According to Le et al, 2014, the chance of re-tearing a shoulder rotator cuff after a repair surgery can go as high as 94%
